6th Grade Parents - Vaccines Needed for 7th Grade
The Ohio Department of Health requires that all students entering 7th grade have the following immunizations:
  • Tdap (must be Tdap, it cannot be Td) vaccine
  • Meningococcal -  Students are required to have 1 dose anytime before entering 7th grade
Your child's health care provider can complete the form below OR you can simply send in a copy of your child's vaccine record.  We are asking that vaccines be on file with the school asap, but no later than September 30, 2020.
Forms or vaccine records can be sent in to the school via fax 937-434-6692, email  sullmer@stcharleskettering.org  or dropped off at the main office. The preferred method is an email your child's vaccine record to  sullmer@stcharleskettering.org
We know that your child probably already received these vaccines, but we still need to have them on file at school. If you would like to use the vaccine form, please go to:   Vaccine Form for 7th Grade 2020-2021
If you have questions, you may contact the school nurse at 433-3746, 937-660-4266, or  sullmer@stcharleskettering.o rg
Wellness Tips for Tweens/Teens

1. Stick to a routine. Go to sleep and wake up at a reasonable time, write a schedule that is varied and includes time for work as well as self-care.

2. Dress for the social life you want, not the social life you have. Get showered and dressed in comfortable clothes, wash your face, brush your teeth. Take the time to do a bath or a facial. Put on some bright colors. It is amazing how our dress can impact our mood.

3. Get outside at least once a day, for at least thirty minutes.  It is amazing how much fresh air can do for spirits.

4. Find some time to move each day, again daily for at least thirty minutes. If you don’t feel comfortable going outside,
there are many YouTube videos that offer free movement classes, and if all else fails, turn on the music and have a dance party!

5. Reach out to others, you guessed it, at least once daily for thirty minutes. Try to do FaceTime, Skype, phone calls,
texting—connect with other people to seek and provide support.
