NEW THIS YEAR:
We have adapted a NEW policy for the administration of acetaminophen and ibuprofen at school. Acetaminophen (Tylenol) and ibuprofen (Motrin, Advil) can NOW be given at school with parental consent ONLY. It will no longer need a doctor's/provider's signature. All other OTC medications and prescriptions medications will still need a parent AND a doctor's/provider's signature.
All other rules will still apply:
  • Medicine can only be brought in by an adult (students are NOT permitted to carry medicines on them)
  • Medicines will be kept in the clinic with proper documentation
  • Medicines will be in the original container
  • Medicines will be picked up at the end of the school year or it will be discarded.

Below you will find the NEW form for OTC Medications - acetaminophen and ibuprofen ONLY and our OLD Medication form for all other medications.
