6th Grade Parents - Vaccine Requirements
Thank you to all everyone who has turned in vaccine records. We are still missing a few. If we have NOT received your child's vaccine record, you will receive an email this week.
The Ohio Department of Health requires that all students entering 7th grade have the following immunizations:
- Tdap (must be Tdap, it cannot be Td) vaccine
- Meningococcal - Students are required to have 1 dose anytime before entering 7th grade
Forms went home a few weeks ago. Your child's health care provider can complete this form OR you can simply send in a copy of your child's vaccine record. We are asking that vaccines be on file with the school no later than 5/25/2018.
We know that your child probably already received these vaccines, but we still need to have them on file at school.
